The Bridge Program

Preparing people to participate meaningfully in change
The Bridge Program is a structured six-session readiness and responsivity program for people preparing to engage with intervention, treatment, supervision or community support.
It helps participants understand how everyday conditions influence regulation, attention, decision-making and behavioural follow-through, while also addressing selected criminogenic needs where they intersect with substance use, relationships, self-regulation, risky routines and behavioural pressure points.
The program is evidence-informed and grounded in responsivity, desistance theory, Neuroplasticity Focused Interventions and PRISMS. Its primary purpose is to strengthen readiness for focused change work while increasing participants’ capacity to recognise and respond to conditions associated with offending, relapse, breach and harmful behaviour.
The Bridge Program may be suitable for people:
- Preparing to begin intervention or treatment
- Struggling to engage consistently with support
- Preparing for release or community transition
- Living with unstable routines or regulation conditions
- Requiring stronger help-seeking and service-navigation skills
Converging Practices supports organisations to pilot, adapt and implement The Bridge Program through facilitator training, implementation planning and evaluation support.

Strengthening the everyday foundations that support safer choices
Pillars of Change is a flexible six-session readiness and regulation program designed for sentenced and unsentenced participants who may be reluctant to engage in more focused treatment.
Each session explores one domain of everyday functioning, including sleep, helpful routine, movement, intake, relationships and stress management. Participants examine how these conditions influence regulation, decision-making and periods of risky behaviour without being required to discuss an alleged or proven offence.
The program can be delivered as a rolling open group, allowing participants to join at different points while still completing meaningful stand-alone sessions.
Pillars of Change may suit people who are:
- On remand or serving a sentence
- Resistant or hesitant about treatment
- Living with unstable routines or regulation conditions
- Experiencing recurring periods of risky or harmful behaviour
- Requiring a practical entry point into reflection and change
Converging Practices supports organisations to pilot, adapt and implement Pillars of Change through facilitator training, implementation planning and evaluation support.

Co-Design a Program.
Working with you to create what your organisation needs.
Programs designed around your cohort, setting and purpose
Converging Practices partners with organisations to develop behaviour change, readiness and practitioner development programs around a clearly defined population, service challenge or practice gap.
We combine your organisation’s operational knowledge with our expertise in Neuroplasticity Focused Interventions, PRISMS, R8 and applied program design. The result is a practical, evidence-informed program that reflects the people you support, the work your practitioners perform and the conditions in which delivery must occur.
We can help you:
- Strengthen an existing program through clearer behavioural logic, improved responsivity, updated activities or enhanced facilitator guidance.
- Create a program around a defined population, behavioural concern, service gap or organisational priority.
- Prepare a program for delivery through facilitator training, local adaptation, implementation planning and evaluation support.
